Notice of Privacy Practices

The U.S. government established a privacy rule (HIPAA) concerning protected health information. This notice describes how medical information about you may be used and disclosed and how you can get access to this information.

Use and disclosure of protected information:

Federal law provides that we may use your medical information (protected health information) for treatment of you without further specific notice to you or written authorization by you. This information may be disclosed to other health care professionals for the purpose of evaluating your health, diagnosing medical conditions, and providing treatment. This may include transmitting information, reports and laboratory studies.

Your medical information may be used to obtain payment for our services without further specific notice to you. We are required to provide third party payers (insurance companies, Workers' comp, no-fault auto insurance) with diagnosis codes for your visit and a description of the services provided.

Your medical information may be used for health care operations such as accounting functions, quality assurance, and claim management with our medical liability insurer.

Disclosure of your health information may be required by law in certain instances without specific authorization by you. This may include public health purposes, to report suspected child abuse, judicial proceedings, law enforcement purposes, to avoid a serious threat to public health or safety and possibly other situations as required by law. Protected health information may be used to report adverse events of medication or medical devices.

Your medical information may be used to send you appointment reminders and to advise you of new treatment and management programs for your medical condition.

You have the right to inspect and obtaining a copy (at a reasonable fee) of your protected health information for as long as we maintain your records.

You have the right to restrict or limit the use of your protected health information. This might include release of information to family or friends. This practice retains the right to deny you further treatment if it is felt that this restriction will adversely affect the health care provided at this office.

You have the right to make reasonable requests regarding methods of communicating with you in a confidential manner, such as leaving phone messages for you or on answering devices.

You have the right to request amendments to your medical information. Such requests must be in writing and must state the reason for requested amendments. We reserve the right to disagree with any requested amendments.

You have the right to request an accounting of any disclosures we make of your medical information.

We are required by law to maintain the privacy of protected health information and to provide you with this notification.

We reserve the right to revise this notice and to make a new notice effective for all protected health information we maintain. If the need arises, a revised notice will be posted in our facility and copies will be available.

If you choose to complain about violations of your privacy rights you have the right to file a complaint with the Secretary of the Department of Health and Human Services of the United States. You may also file a complaint with us. Complaints should be directed to Robert Feld, M.D., 205 East Main Street, Huntington, New York 11743. No retaliatory action will be taken against you for any complaint you may make.
